Background of Macmillan Social Prescribing Service

Family Action were awarded funding from Macmillan Cancer Support to implement a social prescribing service based within GP practices in the City of London and Hackney to support people with cancer.

This is an extension of our existing social prescribing service within City and Hackney that supports people with long-term health concerns, or those experiencing social isolation or social issues, to access activities, social groups, services, and advice and information based on their needs and interests.

What is Macmillan Social Prescribing Service?

Macmillan Social Prescribing Service promotes good health and well-being by linking those who seek support to local activities, social groups, services, and information based on individual interests and needs.

Macmillan social prescribers work with people to identify needs through electronic Health Needs Assessment (eHNA) that are having an impact on their quality of life.
