Clinical Pharmacists and Pharmacy Technicians

Clinical Pharmacists and Pharmacy Technicians are important members of the multidisciplinary team here at Shoreditch Park and City PCN.
Our Clinical Pharmacists provide structured medication reviews for our patients in care homes, those with long term conditions, those severely frail and those on high risk medication prescriptions. Our Pharmacy Technicians manage medication queries and drug reconciliations, and conduct patient reviews.
Pharmacists will be looking at the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with multiple conditions, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
Our Clinical Pharmacist Team is available at all the practices in Shoreditch Park and City PCN.
