Care Coordinators

What is a PCN Care Coordinator?

Care Coordinators work as part of a multidisciplinary team in primary care to identify and provide additional assistance to people in need of proactive support. They work with a case load of patients, acting as a central point of contact to ensure appropriate support is made available to patients and their carers where relevant. Care coordinators work with people individually, building trusted relationships and focusing on what matters to the person in order to support them to develop a personalised care and support plan. They also support people in preparing for or following up on consultations with health care professionals to help them to be actively involved in managing their care and supported to make choices that are right for them.
