The Community Navigation Team at Shoreditch Park & City Primary Care Network has transformed how residents connect with care, community and support.

Born from listening to patients and frontline staff, this team created a simple but powerful idea — one single point of access that links people to the right help at the right time. Whether that support is medical, social, or emotional, the team ensures nobody gets lost in the system.

Bringing together social prescribers, health coaches, care coordinators, mental health practitioners, financial advisers and voluntary sector partners, the team works as one, offering holistic, compassionate, person-centred care. Each referral is reviewed collaboratively, ensuring safe, timely, and appropriate support that reflects what matters most to the individual.

The impact has been: safer care, better outcomes, reduced duplication, fewer unnecessary GP appointments and hospital admissions, and improved patient and staff experience. Residents now experience faster, warmer connections to the services and people that help them live well.

The model has already inspired other Primary Care Networks to replicate it — proof that when systems join forces around people, communities thrive.
